Me: If a bicarb of 6 and an anion gap of 35 doesn’t get you excited you shouldn’t be a nephrologist.
My fellow: Yeah, its the ST-elevation MI of nephrology.

Great point; I was looking for something like this to push me over the edge on my decision. After 27 years of practicing Nephrology, a bicarb of 6 and AG of 35 no longer gets me excited. Instead the emotions are sadness, fear, and weariness from having been there and seeing that and having the outcome in so many cases be bad. I have finally figured out that eventually all this sadness that gets buried as we slog along, comes out eventually and says in a loud voice, “NO MORE!”. Thanks.
